Open Question: I got temporary credit from Bank of America. They ask me to file this document?

I called Bank of America yesterday about 4 transactions in my bank account that I did not make (those scams). For now they gave me part of the temporary credit and they mentioned about sending them this document with this address in phoenix, arizona otherwise my temporary credit will cancel. Is this document going to be sent online or it is going to be sent through mail?
